1: Legalization of Signature (General Power of Attorney)
by mail: Please refer to the information available under Power of Attorney.

 

2: Affidavit:

  • The affidavit must be signed by the applicant and two witnesses.
  • The affidavit must be notarized by a notary public and include the following statement on the page bearing the signature of the applicant and the two witnesses:” Before me appeared personally Mr/Mrs/Ms…….. and witnesses Mr/Mrs/Ms……… and signed this document, this day …/…./….”.
  • The affidavit must be authenticated by the Secretary of State (please include a link to the list of secretaries of state websites), or the relevant authenticating authority.
  • A self- addressed stamped return envelope.
  • A copy of the affidavit and notarized copies of applicant and witnesses valid ID cards.

N.B: The amount is payable at the Consulate General of Lebanon in Los Angeles.

3: Life Certificate:

  • A letter signed by the applicant and notarized by a notary public.
  • The letter must be authenticated by the Secretary of State.
  • A notarized copy of the applicant’s Lebanese I.D. card or valid Lebanese passport.
  • A stamped self-addressed return envelope.
  • A copy of each original document.

3: Life Certificate:

4: School, College, and University Certificates and Transcripts:

  • The certificate/transcript must to be notarized by the school, college, or university notary.
  • It then must be authenticated by the Secretary of State. (please include link).
  • A stamped self- addressed return envelope.
  • Please note that the school, college, or university must be accredited in the United States, and a proof of accreditation must be provided with the application.
  • A copy of all original documents.

5: Medical Report:

  • The medical report has to be notarized then authenticated by the Secretary of State.
  • A copy of Lebanese I.D. card. • A copy of all original documents.

Note: Government employees and their family members must provide a copy of their Lebanese Governmental employment I.D. card, and a copy of their Lebanese I.D. card. The formality is free of charge for government employees.

6: Palestinian Vital Records:

  • The original document.
